What is Perimenopause?

"Why does one stage of life suddenly seem to affect every part of my body?"

If you've found yourself asking that question, you're not alone.

Many women begin perimenopause expecting little more than irregular periods and the occasional hot flush. Instead, they find themselves lying awake at three in the morning, forgetting simple words, feeling anxious for no obvious reason, or wondering why they no longer feel like themselves anymore.

It can feel as though your body has started working against you.

It hasn't.

Your body is simply trying to adapt to one of the most significant biological transitions of your adult life.

Understanding that transition is the first step towards understanding yourself again.

So, what is perimenopause?

Perimenopause is the natural stage of life before menopause. It is the gradual transition from your reproductive years towards the end of your reproductive life.

Although we often talk about perimenopause as if it is simply "the time when oestrogen falls", the reality is far more interesting and far more complex.

Perimenopause is not just about your ovaries.

It is about your body's entire hormonal communication system adapting to change.

Your ovaries are gradually producing hormones less predictably than they once did. Instead of following the regular monthly rhythm your body has relied upon for decades, hormone production becomes increasingly irregular. Some months your ovaries may release an egg. Other months they may not. Hormone levels can rise sharply one week and fall the next.

This unpredictability is one of the defining features of perimenopause.

Your hormones don't work alone

One of the biggest misconceptions about perimenopause is that it only involves two hormones—oestrogen and progesterone.

These hormones are undoubtedly central to the process, but they are part of a much larger network known as the endocrine system.

The endocrine system is made up of glands throughout your body that produce hormones. These chemical messengers travel through your bloodstream, carrying instructions that help different organs communicate with one another.

Imagine an orchestra.

Each musician plays a different instrument, but they all follow the same piece of music. If one section suddenly changes tempo, the rest of the orchestra has to adjust.

Your hormones work in a similar way.

Your brain, ovaries, adrenal glands, thyroid and pancreas are constantly exchanging information. They do not work independently; they work together to help maintain balance throughout your body.

As the ovaries begin to send less predictable hormonal signals during perimenopause, the rest of the endocrine system continually adapts.

This is one reason why perimenopause can affect so many seemingly unrelated parts of your body.

Why does it affect so much more than your periods?

Many women are surprised to discover that perimenopause can influence sleep, memory, mood, joints, skin, bladder function, body temperature and even how resilient they feel when dealing with stress.

That is because hormones do far more than regulate reproduction.

For example:

  • Oestrogen influences brain function, blood vessels, bones, skin and many aspects of metabolism.
  • Progesterone plays an important role in the menstrual cycle and also has calming effects within the brain.
  • Cortisol, often called the body's primary stress hormone, helps us respond to physical and emotional challenges.
  • Melatonin helps regulate our sleep–wake cycle.
  • Insulin regulates blood glucose, providing the energy every cell in the body needs to function.
  • Thyroid hormones help control how quickly our body uses energy.

These hormones are not all changing because of perimenopause, but they are part of the same interconnected communication network. As ovarian hormones fluctuate, the body is continually adjusting to maintain balance.

This helps explain why the effects of perimenopause can extend far beyond the reproductive system.

The brain is part of the story too

For many years, menopause was viewed almost entirely as a reproductive event.

Research now tells us that it is also a neurological one.

The brain contains receptors that respond to oestrogen in areas involved in memory, sleep, mood, attention and temperature regulation.

As hormonal signalling changes, these brain networks adapt.

Scientists believe this helps explain why many women notice changes in concentration, emotional resilience, sleep and cognitive function during perimenopause.

This does not mean your brain is failing.

It means your brain is adapting.

For many women, simply understanding that can be enormously reassuring.

Why do symptoms come and go?

One of the most frustrating aspects of perimenopause is its unpredictability.

You may feel perfectly well for several weeks before symptoms suddenly return.

This happens because hormones do not decline in a straight line.

Instead, they fluctuate.

Some days your body receives one pattern of hormonal signals.

A few days later, those signals may be completely different.

Your brain and the rest of your body are constantly responding to those changing messages.

That is why symptoms often come and go rather than following a steady progression.

Every woman's experience is unique

No two women experience perimenopause in exactly the same way.

Some notice very few symptoms.

Others find that poor sleep becomes their biggest challenge. For someone else it may be anxiety, joint pain or brain fog.

Your genetics, general health, lifestyle, medical history and the way your body responds to hormonal change all influence your experience.

There is no "right" or "wrong" way to experience perimenopause.

Understanding is empowering

Perimenopause is a natural stage of life, but that does not mean you simply have to endure it.

Understanding what is happening inside your body allows you to recognise patterns, ask informed questions and explore the options available to you, whether that involves lifestyle changes, medical treatment, complementary therapies or a combination of approaches.

Knowledge doesn't provide all the answers.

But it does replace uncertainty with understanding.

And that is often where confidence begins to return..
